Return to work referral form
This form should be used by medical professionals who want to support a stroke survivor back into the workforce through our Return to Work service.

About this service
The service is funded by the Ministry of Social Development (MSD) and is free for New Zealand citizens and permanent residents, or Australian citizens, aged between 15 and 64. This service is available nationwide.
Tips for referrers to support a successful return to work:
- Encourage referral and engagement with our service as soon as possible after a stroke.
- Let the person / whānau member know that many people have a successful return to work after a stroke.
- Suggest not making any big decisions about work until they have spoken to a Community Stroke Navigator (0800 STROKE).
- Provide information on driving after a stroke.
- If a person is not eligible for our service, consider a reaching out to a Community Stroke Navigator to give basic support, information and guidance.
